For Parents

• Please provide the student with a copy of your health insurance card including the card holders’ date of birth and social security number.
• We are in-network with most major health plans; please verify with your insurance company; pre-authorization may be required by your insurance carrier.
• The Campus Health Clinic cannot fill prescriptions; however, there are several local pharmacies in the area.
• If you see a charge for the Campus Health Clinic on your student’s monthly invoice, the clinic is not obligated to release information to the parent on the “reason” for the visit. This information is protected health information since the student is over the age of 18.
