How do I begin care with Southeast Home Health?
- Talk to your physician to see if home health care is right for you.
- If it is, tell your physician that you want your home health through Southeast.
- Call our office for more information. If you are hospitalized, ask to talk to a social worker or case manager.

Home Health staff will contact your insurance company in order to determine your particular coverage and will inform you before beginning services.
