We can assist the homebound by:
(Homebound patients are those that leave home infrequently
and with assist or effort)
- Assessing medication complicance and effectiveness
- Instructing on disease management and medication
changes with associated nursing assessments
- Assessing pain control
- Assessing home safety
- Assisting with community resource needs
- Providing physical, occupationa and/or speech
therapy to restore patients ADL's
- instruct caregivers in care for patient
- instruct on use of DME (i.e. walkers,
quad canes, Hoyer lift, cruches)
- Wound care
- Decubitus care
- Catheter Care - Foley, suprapubic, or IV
catheter care
- Ostomy Complications/instruction (i.e. colostomy,
ileostomy, ileoconduit, peg tub, G tubes,
etc.)
- Impaction followup care
