Home Health Beneficial for Patients Upon Discharge

We can assist the homebound by:
(Homebound patients are those that leave home infrequently and with assist or effort)

  • Assessing medication complicance and effectiveness
  • Instructing on disease management and medication changes with associated nursing assessments
  • Assessing pain control
  • Assessing home safety
  • Assisting with community resource needs
  • Providing physical, occupationa and/or speech therapy to restore patients ADL's
    • instruct caregivers in care for patient
    • instruct on use of DME (i.e. walkers, quad canes, Hoyer lift, cruches)
  • Wound care
  • Decubitus care
  • Catheter Care - Foley, suprapubic, or IV catheter care
  • Ostomy Complications/instruction (i.e. colostomy, ileostomy, ileoconduit, peg tub, G tubes, etc.)
  • Impaction followup care
