- Talk to your physician to see if home health
care is right for you.
- If it is, tell your physician that you want
your home health through Southeast.
- Call our office for more information. If you
are hospitalized, ask to talk to a social worker
or case manager.

Home Health staff will contact your insurance
company in order to determine your particular
coverage and will inform you before beginning
services.

that is Medicare certified, Medicaid approved,
