Depending on your medical condition,
the following services are available through home
health:
Skilled Nursing: A Nurse will
visit to assess your medical condition and the
medicines you are taking and will provide the
necessary care ordered by your physician. The
nurse will also teach you or a family member/caregiver
about your medicines and work with you to make
you as independent as possible. A registered nurse
is available 24 hours a day, 7 days a week.
Home Health Aids: Nursing Assistants
will visit to provide personal care to you such
as bathing and grooming in order to make you more
comfortable and, when possible, assist you in
regaining your independence in this area. A Registered
Nurse assesses an infant enrolled in Southeast
Home Health.

| Physical Therapy:
A Physical Therapist will visit if you are in
need of rehabilitation from an injury or surgery,
especially bone and joint surgery. The therapist
will work with you and teach you the necessary
exercises that will increase your strength and
ability to get around.
Speech Pathology: A Speech Pathologist is available
for those having difficulty communicating as a
result of illness or injury.
Occupational Therapy: An Occupational
Therapist is one who will work with you to make
sure that you have the right assisting devices
and equipment to help you try to return to your
previous lifestyle.
Medical Social Work: The Social
Worker is available if you are in need of additional
community resources, need assistance in applying
for Medicare or Medicaid, are having financial
difficulties, want to complete an advance directive
or living will, are interested in admission to
a nursing home, or have counseling needs.
Nutritional Counseling: A Dietitian
is available for consultation if you have a special
diet or dietary concerns.
