Checking Out
After your doctor orders your release from the Hospital, you or one of your family members will do the following:
Go to the Nurses’ Station on your floor to receive follow-up appointments for physician or Hospital services and special care or post-Hospital instructions. If all insurance forms have been completed and if there are no unresolved questions about the billing for your Hospital stay, you may leave directly from your floor. If you have questions about insurance or your Hospital charges, a representative of Southeast’s Business Office will be glad to assist you.
Special Notes About Your Dismissal:
- It is Hospital policy that all patients being discharged must be escorted by Hospital personnel.
- There may be several personal items that you may take home with you. If you have questions about whether equipment or supplies are yours to take, please ask a nurse.
- The Lobby is the designated departure area for patients. The person picking you up should park in front of the Lobby only when you are ready to be dismissed.

Planning for Dismissal
Some patients leaving the Hospital may find that they have special needs either at home or that they need to be transferred to another facility. If you have special needs, Southeast’s Case Management Department is available to help you with any necessary arrangements.
While Hospital care is designed to relieve physical illness, it may also produce many other concerns. The Social Workers in the Hospital’s Case Management Department can help ease many of those concerns for you and your family by answering questions you may have about financial assistance, transportation services, and lodging for your family while you are a patient.
Before you are dismissed from the Hospital, a Social Worker may visit with you to discuss the options open to you for continued care as well as any costs involved.
Post-Hospital Care May Include Any Of The Following:
- Returning to your own home with help (Meals on Wheels, Home Health, private duty nursing, homemaker services, etc.)
- Going to a relative’s home.
- Obtaining durable medical equipment (a hospital bed for in-home use, walkers, bedside commodes, etc.)
- Going to another hospital, boarding home, residential care facility, nursing home, Veterans Administration hospital, or rehabilitation center.
